Offline tQuest Forms

Downtime’ or ‘offline’ tQuest Forms

A new form has been created to ensure GPs can still order pathology tests when they cannot access the Synnovis version of tQuest.

The forms standardise other similar paper forms and will mean that GP practices in Bexley, Greenwich, Lewisham, Bromley, Southwark and Lambeth can still order pathology tests when systems are not working for any reason.

This could be as a result of an issue which prevents people from sending electronic requests at their own organisation or in the event of an outage or planned downtime for system maintenance.

Electronic ordering remains the preferred and quickest way of being able to order pathology tests, but the offline forms will enable tests to be ordered during those exceptional times when the system is temporarily unavailable.

There are three different forms to use depending on the type of test being requested:

  • Blood sciences / Biochemistry
  • Virology
  • Microbiology

Copies of the forms can be downloaded using the buttons below.

EMIS versions of the forms, which can be uploaded directly to practice systems, will be available soon via this web page.

DXS versions can be found in the ‘NHS South East London CCG’ folder on the DXS platform, by selecting ‘Pathology offline’ and then ‘TQuest Synnovis’.
